1. Introduction

Ethos Pediatric Therapy (“we,” “us,” or “our”) is committed to protecting the privacy of every person who visits our website or contacts us. This Privacy Policy explains what information we collect, how we use it, how we protect it, and what rights you have regarding that information.

This policy applies to the Ethos Pediatric Therapy website located at https://ethospedstherapy.com/ (the “Site”). It does not apply to information collected during clinical care, which is governed by our clinical Notice of Privacy Practices provided to patients and families at the time of service.

By using this Site, you agree to the practices described in this Privacy Policy.

3. Information We Collect

We collect two categories of information through this Site: information you provide to us voluntarily, and information collected automatically when you visit the Site.

3a. Information You Provide

Appointment Request Form (JotForm)

When you submit an appointment request through our JotForm link, you may provide:

  • Your name and your child’s name
  • Contact information (email address, phone number)
  • Your child’s age and general reason for seeking services
  • Insurance Carrier
  • Any additional information you choose to share in an open-text field

This information is used solely to respond to your inquiry and determine whether our services may be a good fit for your family. It is not used for marketing purposes and is not sold or shared with third parties, except as described in Section 5 of this policy.

Important note about medical information: We do not collect, store, or process protected health information (PHI) through this website. Our appointment request form is not a clinical intake form and does not request medical records, diagnoses, insurance information, or treatment history. Information collected through our website is not subject to HIPAA as website data; however, any clinical information shared during the course of care is handled separately and in full compliance with applicable federal and state law, including HIPAA.

3b. Information Collected Automatically

When you visit our Site, certain information is collected automatically through Google Analytics and Google Search Console (via Google Site Kit), including:

  • Pages visited and time spent on each page
  • General geographic location (city/region level — not precise location)
  • Device type, browser type, and operating system
  • How you arrived at the Site (search engine, direct link, referral)
  • Search terms used to find the Site (via Google Search Console)

This information is collected in aggregate and is used to understand how visitors use our Site, improve its content and usability, and measure the effectiveness of our online presence. It does not identify you personally.

4. Cookies and Tracking Technologies

Our Site uses cookies — small text files stored on your device — to support the functionality of Google Analytics. These cookies do not collect personally identifiable information. They assign an anonymous identifier to your browser session, which allows Google Analytics to track aggregated usage patterns over time.

You may disable cookies through your browser settings. Doing so will not affect your ability to use our Site or submit an appointment request, though some analytics features will no longer function.

For more information on how Google uses data collected through its analytics tools, please visit Google’s Privacy & Terms at policies.google.com/privacy.

5. How We Use and Share Your Information

We use the information collected through this Site for the following purposes:

  • To respond to appointment requests and communicate with prospective patients and their families
  • To understand Site usage and improve the quality and usefulness of our online content
  • To maintain and improve our online presence through search performance data from Google Search Console

We do not sell, rent, or trade your personal information. We do not share your information with third-party marketers or advertisers.

Information may be shared in the following limited circumstances:

  • With service providers: Google (Analytics, Search Console, Forms) processes data on our behalf. Google’s data practices are governed by Google’s Privacy Policy and Terms of Service.
  • As required by law: We may disclose information if required to do so by law, court order, or governmental authority.
  • To protect rights or safety: We may disclose information when necessary to protect the safety of our staff, patients, or the public.

6. Children’s Privacy

Ethos Pediatric Therapy serves children ages 2–18, but our website is directed at parents and guardians — not at children themselves. We do not knowingly collect personal information directly from children under the age of 13 through this Site.

All appointment requests and communications are expected to be submitted by a parent or legal guardian on behalf of their child. If you believe a child under 13 has submitted personal information through our Site without parental consent, please contact us immediately at plymouthpeds@ethosptmi.com and we will take steps to remove that information.

This Site is operated in compliance with the Children’s Online Privacy Protection Act (COPPA), which governs the online collection of information from children under 13.

7. Data Retention

Information submitted through our appointment request form is retained for as long as necessary to respond to your inquiry and for a reasonable period thereafter for record-keeping purposes. You may request deletion of your information at any time by contacting us (see Section 9).

Aggregated analytics data collected by Google Analytics is retained in accordance with Google’s data retention settings, which we have configured to 14 months.

8. Data Security

We take reasonable precautions to protect the information collected through this Site. Our appointment request form is transmitted through Google Forms using encrypted connections (HTTPS). We do not store credit card numbers, insurance information, or clinical records through this website.

No method of electronic transmission or storage is 100% secure. While we strive to use appropriate means to protect your information, we cannot guarantee absolute security. If you have concerns about submitting sensitive information online, we encourage you to contact us by phone at (734) 463-3007 ext. 3.

9. Your Rights and Choices

You have the right to:

  • Access the personal information we hold about you
  • Correct inaccurate information
  • Request deletion of your information, subject to any legal obligations to retain it
  • Opt out of analytics tracking by disabling cookies in your browser or using browser privacy settings

To exercise any of these rights, please contact us at plymouthpeds@ethosptmi.com or by phone at (734) 463-3007 ext. 3.

Michigan residents may have additional rights under Michigan law. [CONFIRM with attorney — Michigan does not yet have a comprehensive consumer privacy statute, but this space is evolving.]

10. Third-Party Links

Our Site may contain links to other websites, including the websites of the Ethos family of practices (Ethos Physical Therapy, Ethos Human Performance, and the Regeneration Foundation). The privacy policies of each of those websites are their own, not related to the policies of this website.

11. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes in our practices, legal requirements, or the tools we use. When we make changes, we will update the “Last Updated” date at the top of this page. We encourage you to review this policy periodically.
